Physical Examinations
 
Students registering for Athletics must have a current physical examination on file in the Activities office before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and turned in to the Activities office no later than 3 school days prior to the first day of practice/tryouts.
 

A student-athlete whether an adult or not, must have written permission from parents to participate in school athletics and he/she must have a physical examination (signed by a licensed physician or advanced practice nurse prescriber) every other school year.

*A physical form OR an alternate year card must be turned in each year. * A physical examination received April 1 and thereafter is valid for the following two school years. Physical examinations received before April 1 are valid only for the remainder of that school year and the following school year.

The year opposite of when a physical is needed, an Alternate Year Card must be submitted. Alternate year cards signed after May 1 are valid for the upcoming school year.

The following sports are offered at the middle school: Cross Country and wrestling for grades 5-8, basketball and volleyball for grades 7-8 and track for grades 6-8.
